If you do not wish to use the rubber band or ring method, or if you find that they do not work for you, the two-finger tourniquet might be a good solution. Using a two-finger tourniquet is really quite simple, and is very similar to the erect girth exercise.
Instead of grasping the base of the penis with the thumb and forefinger in an "o" shape, much like holding a baseball bat (we'd hope!) you can tourniquet effectively with only two fingers, or three, your thumb and forefinger from a VERTICAL angle (meaning they are above the penis, so your whole hand is not in the way of your stroking hand. Or you can approach from a horizontal angle, from the right or left side of the penis, with whichever hand you are not using to stroke. This would look more like you are holding a potato chip with your thumb facing up and holding the top of the penis, your forefinger below, both applying pressure to help tourniquet the penis. You can use three fingers if you wish, your middle finger and your forefinger on one side, your thumb on the other. Essentially you are pinching the penis instead of squeezing it with a fisted grip, much like the squeezing of the penis in the erect girth exercise.
The two-finger tourniquet is a useful variation on the tourniquet method, if you can use it properly given penis and hand size. If you cannot, using a rubber band or cock ring may be the best way.

Grip variation can help some men, especially those who have difficulty with the tourniquet grip due to small penis size, or large hand size. Currently the milking exercises (semi-erect girth for example) suggest the use of most of the hand, but in order to get closer to the base for milking, it may be necessary to flip the hand, which looks like so:
